Folds in part

UHC Dual Complete KS-S002 (H5322-029) is not offered under this ID in 2027. The CMS plan crosswalk of October 1, 2026 moves its members into 2 2027 plans, each sold in part of its area (listed below), in the 67 counties where they are sold (1,081 members in September 2026). In its other 1 county (0 members) there is no plan to move into: coverage ends December 31, 2026, and members there need to choose a new plan (counties with fewer than 11 members are not counted). CMS also moves some members into H0169-012, a plan not in the individual 2027 files (an employer or special plan), so it has no page here. The crosswalk also lists part of the plan as terminated without saying which members; your Annual Notice of Change says whether you move to a new plan or your coverage ends.

What changes for its members, plan by plan?

In 2026 the plan cost $51.70 a month with a $9,250 in-network limit and a $2.10 Part B reduction. Each row compares that with one of the 2027 plans its members move into.

Each plan's own page compares every line; your Annual Notice of Change names your 2027 plan; confirm on Medicare Plan Finder.

Which 2027 plans do its members move into?

The crosswalk names the plans but not which member goes to which; the counties show where each is sold, and your Annual Notice of Change names yours.

What can members do, and by when?

If you live in Allen (KS), Anderson (KS), Atchison (KS), Barber (KS), Bourbon (KS), Brown (KS), Butler (KS), Chase (KS), Chautauqua (KS), Cherokee (KS), Clay (KS), Cowley (KS), Crawford (KS), Dickinson (KS), Doniphan (KS), Douglas (KS), Edwards (KS), Elk (KS), Ellsworth (KS), Franklin (KS), Geary (KS), Greenwood (KS), Harper (KS), Harvey (KS), Jackson (KS), Jefferson (KS), Jewell (KS), Johnson (KS), Kingman (KS), Kiowa (KS), Labette (KS), Leavenworth (KS), Lincoln (KS), Linn (KS), Lyon (KS), Marion (KS), Marshall (KS), McPherson (KS), Miami (KS), Mitchell (KS) and 27 more (listed below)

If you do nothing
You are moved into one of the 2027 plans above on January 1, 2027. Your Annual Notice of Change names it.
October 15 to December 7, 2026
Open enrollment: choose another Medicare Advantage plan, or Original Medicare with a drug plan, instead.

If you live in Cloud (KS)

Your coverage
Ends December 31, 2026: none of its 2027 plans is sold where you live.
October 15 to December 7, 2026
Open enrollment: join another Medicare Advantage plan, or Original Medicare with a drug plan. Coverage starts January 1, 2027.
December 8, 2026 to the end of February 2027
A special enrollment period because the plan is leaving: you can still join a plan, but coverage starts the first day of the month after you join, so choose by December 31 to have no gap.
If you do nothing
You are enrolled in Original Medicare when the plan ends, without a drug plan unless you join one (with Extra Help or full Medicaid, Medicare enrolls you in one).
Without a drug plan
Going 63 days or more in a row without drug coverage can mean a late enrollment penalty if you join a drug plan later; joining by the end of February keeps a January and February gap under 63 days, waiting longer does not.
If you have Medicaid or Extra Help
You can switch to a different drug plan, or leave a Medicare Advantage plan for Original Medicare with a drug plan, once a calendar month; the change starts the first day of the next month.
