Type the plan ID from your member card, your Annual Notice of Change or a mailer to see the plan's 2026 and 2027 figures and whether it renews, folds into another plan or ends. The lookup covers 5,412 plans for 2027 and every individual plan of 2026.
Where is the plan ID?
On a Medicare Advantage card the ID is often printed as a contract number and a plan number, for example H5216 and 393, sometimes with a segment number after them. A drug plan's ID starts with S.
Not found? An employer or union plan (plan numbers 800 and above) is not in the CMS landscape file, and plans before 2026 are not in the files this site holds.
