The tables compare CMS's 2025 and 2026 overall Star Rating releases. A positive value means the reported overall rating rose; a negative value means it fell. 50 plans with gains and 50 with declines are shown, ordered by the size of the movement and then service-area reach.

Method and limits
CMS labels these as 2025 and 2026 plan-year ratings, even though the release files are published before the associated coverage year. PlainMedicare joins the two Summary Ratings tables on CMS Contract Number and keeps only plans with a reported overall rating in both releases. The ranking de-duplicates the county-level source into one contract-plan row before sorting, so broad service areas do not create repeated entries.
